Submitting this agreement does not guarantee acceptance into the EXCEL Club. All membership agreements will be reviewed by the EXCEL Club Board, and applicants will be considered through our membership review process. Once a decision has been made, the New Member Chair will contact you regarding your application status and any next steps.

Photo & Media Release

I give permission for photographs, videos, and audio recordings of the student to be used by the Excel Club of Fontana, Exchange Club of Fontana, Fontana Unified School District (when applicable), and partner organizations for newsletters, websites, social media, promotional materials, and recognition of club activities.

Responses should reflect the student's own thoughts and experiences. Parents may assist, but responses should be written primarily by the student.
